Care Coordinator
Overview Guardians Recruitment – recruiting on behalf of a domiciliary care provider Guardians Recruitment are recruiting on behalf of a growing, person-centred domiciliary care provider for an experienced Care Coordinator. This is a key hire to support service delivery and drive growth across Chippenham and the surrounding area (ideal for candidates living locally). The service currently delivers around 2,000 care hours per week, including 2 live-in care packages, and is expanding rapidly in the Chippenham area. The role You’ll be responsible for coordinating care delivery day-to-day, managing rotas, supporting carers and clients, and ensuring high standards of compliance and service quality. This role will also contribute directly to the growth and development of the area.
What you'll be doing
- Key responsibilities:
- Scheduling & rota management
- Prepare, manage and distribute rotas accurately and on time
- Allocate visits efficiently, considering staff availability, client preferences and continuity of care
- React quickly to changes (sickness, urgent cover, new packages) and adjust rotas accordingly
- Maintain accurate scheduling records on the care management system (PASS preferred)
- Monitor daily alerts and take prompt action where required
- Manage annual leave and maintain safe staffing levels
- Participate in the on-call rota (1 night midweek / 1 weekend in 4)
- Client & carer satisfaction
- Build strong relationships with clients and families
- Support carers with clear schedules and accurate information
- Resolve concerns promptly and professionally
- Manage medication changes quickly, updating care plans and communicating changes to carers
- Compliance & monitoring
- Monitor ECM throughout the day to confirm attendance and continuity
- Ensure carers remain in calls for at least 95% of scheduled duration and address shortfalls
- Manage alerts relating to missed calls, medication discrepancies and call overruns
- Support audits, documentation reviews and CQC readiness
- Complete the Wiltshire Council Capacity Tracker weekly (preferably Thursdays)
- Monitor and manage incident/accident reports raised via PASS, ensuring investigations and actions are completed appropriately
- Operational support & growth
- Work closely with the management team to support sustainable growth
- Support recruitment, onboarding and training of new care staff as required
- Identify improvements to efficiency, quality and communication
What we're looking for
- Essential requirements
- Experience in care coordination / scheduling / admin within health & social care
- Knowledge of domiciliary care operations and CQC requirements
- Confident using care management/scheduling software (PASS preferred)
- Strong organisation, attention to detail and ability to work under pressure
- Excellent communication and relationship-building skills
- Full UK driving licence and access to a vehicle
- Desirable
- NVQ Level 3 in Health & Social Care (or working towards)
- Experience in a growing home care business
